Abstract
Localized colitis cystica profunda, showing evidence of proctitis as the pathogenetic mechanism, occurred in a 27-year-old man. Local excision of these midrectal lesions via posterior proctotomy is a satisfactory approach to their management. Awareness of the entity on the part of surgeons and pathologists will prevent needless radical surgery.
