Complications in the Application of the Inferior Vena Cava Umbrella Technique

Abstract

In eight cases the Mobin-Uddin intracaval umbrella filter has been attempted or applied for the control of recurrent pulmonary embolic disease, or both. Each of the patients was critically ill, thus precluding the direct operative approach on the vena cava as the primary method of choice. Various complications included (1) insertion in the right common iliac vein, (2) failure of the umbrella to open (insertion in the right renal vein), (3) damage to adjacent structures during insertion in the jugular vein, (4) inability to insert the catheter due to an anomalous venous system, and (5) failure to insert the instrument into the jugular vein due to the vein's small size. Despite such problems, we feel the technique is useful in selected circumstances.
