Preferential Laparoscopic Live Donor Left Nephrectomy in Women of Childbearing Age

It was with great interest that I read the recent ARCHIVES article by Troppmann et al.1 A growing body of literature now attests to the advantages and safety of laparoscopic live donor nephrectomy. However, the advent of this procedure has changed clinical practice, whereby left kidneys with multiple renal arteries are often preferentially removed for transplantation rather than right kidneys with single renal arteries. The effects of this change in kidney selection previously had not been examined. The authors should be congratulated for their careful study demonstrating that this practice does not have any apparent deleterious effects on the recipient.
Many surgeons, however, still possess lingering concerns about the safety of this practice in female donors of childbearing age because of the supposed greater risk of right ureteral obstruction during pregnancy.
