Cost-Reduction Model for Treatment of Pancreatic Leak Following Distal Pancreatectomy

I read with great interest the article by Rodriguez et al,1 "Implications and Cost of Pancreatic Leak Following Distal Pancreatic Resection," in the April issue of the ARCHIVES. This article described a cost-containment model in the management of pancreatic leak (PL) following distal pancreatectomy. I understand very clearly that the authors were not interested in identifying in this article the preoperative and intraoperative risk factors that lead to PL and the intraoperative techniques that may prevent PL after distal pancreatectomy. Despite this aim, the invited critics brought up the issue of how to prevent this complication. It is a fact that if there is no PL following distal pancreatectomy, then there is no need for such a model and this article. On average, it costs about $3000 to publish a scientific medical article.
