I thank Drs Badruddoja and De Waele for their interest in my article. Since its presentation at the New England Surgical Society in 2005, I have received numerous responses, which range from strongly agreeing to passionately disagreeing with the article. However, I noticed that most of the critique was not directed toward the design or the setup of the article but more with the "idea" that an article is questioning a surgical practice that started more than 100 years ago.
During the preparation of the manuscript, I did not find any articles showing an increased rate of complications when surgery for appendicitis is delayed for 12 hours. Most articles studying the negative impact of late appendectomy concentrated on delays of several days, usually caused by the patients' delay in seeking medical attention.
