Treatment of Stage IVA Hepatocellular Carcinoma

In the June 2008 issue of the Archives, Chirica et al1 report outcomes of resection in patients with stage IVA hepatocellular carcinoma (HCC) according to the fifth edition of the American Joint Committee on Cancer/International Union Against Cancer (AJCC/UICC) staging classification. The authors emphasize the limitations of this classification to predict survival after resection of bilobar nodules.

This classification has actually been replaced with a new one that addresses this limitation. In 2002, the International Cooperative Study Group on HCC proposed a simplified system. They classified HCC with multiple nodules, regardless of lobar distribution, as stage II (no tumor >5 cm) or stage IIIA (any tumor >5 cm).2 The current (sixth) edition of the AJCC/UICC classification, published in 2002, reflects these changes.3-4
