Few diseases approached from a surgical standpoint offer so many interesting possibilities for individual ingenuity and at the same time oppose so many difficulties to rational treatment as does portal cirrhosis with ascites. The stage of the disease under discussion in most instances probably is the last stage, and the surgeon's efforts are directed toward the correction of this particular manifestation, though he is not given an opportunity to affect the disease itself. The literature on the subject is remarkably voluminous when one considers the comparative rarity of the condition and also the fairly brief period—now thirty years—during which surgical operation has been used as a method of treatment. Isolated reports of cases in which treatment consisted of the various surgical procedures available appear with considerable regularity, and in the main these reports seem to indicate a certain justification for surgical intervention.
