Gastric Carcinoma Treated with Abdominothoracic Total Gastrectomy

Removal of the tumor-bearing organ together with the regional lymphatic system is a principle in carcinoma surgery. This was not sufficiently clearly formulated when Conner,2 in Cincinnati, made the first total gastrectomy, in 1884. His patient died on the operating table, and the first total gastrectomy to succeed was performed by Schlatter16 in Switzerland in 1897. A classic description of the lymph drainage of the stomach was given by Jamieson and Dobson,8 in 1907, and subsequently a series of detailed investigations have been carried out in this field—in this country by Eker *—which forms the basis of block dissection in total gastrectomy.
