Despite the large variety of procedures devised to collapse the tuberculous lung, none has proved to be entirely satisfactory. Exhaustive reviews1-6 of such techniques are available and will not be repeated here. This article describes a new technique for attaining pulmonary collapse by simple purse-string inversion of the parietal pleura with collapse of the diseased upper portion of the lung via an extrapleural approach. Experience with this procedure in 105 patients during a 7-year period is reviewed.
Selection of Patients
The usual criteria for pulmonary collapse therapy have been used in the selection of patients for apicoplasty in this study. In general, these were consecutive cases of proved pulmonary tuberculosis among patients (Chinese, Malays, Indians, and others) mainly from the Tan Tock Seng Tuberculosis Hospital, Singapore, between June, 1953, and May, 1960. Each patient was judged incurable by chemotherapeutic or other medical means at combined medical and surgical staff
