Retroperitoneal Fibrosarcoma Without Hypoglycemia

THE EXTREME rarity of retroperitoneal fibrosarcoma warrants the report of all cases. Review of the English literature reveals only nine authentic ones and all of these were associated with severe degrees of hypoglycemia (Table 1). The case herein reported is unique in that it is the first in which hypoglycemia was consistently absent, despite the fact that the patient was followed for more than six years and had next to the largest tumor on record. This raises the question as to the true incidence of association of hypoglycemia with retroperitoneal fibrosarcoma, which heretofore has been 100%.

The salient features in the present case are those of a 65-year-old woman with massive recurrent retroperitoneal fibrosarcoma extending over a period of six years, during which time she underwent three operative excisions and a course of chemotherapy. The tumor became increasingly anaplastic but never produced hypoglycemia.
